⚖️ Forensic Referrals
For participants transitioning from forensic psychiatric units, corrections, or under SACAT orders. Include CTO conditions and FMHS details in the referral.
  • Pre-placement feasibility assessed by COO + BSP
  • Forensic Readiness Pack prepared before move-in
  • Multi-agency transition meeting coordinated by us
🧠 Mental Health Referrals
For dual diagnosis, psychosocial disability, and post-acute psychiatric discharge. Include treating psychiatrist and CTO details where applicable.
  • BSP engaged before admission
  • Medication plan confirmed with RN before first shift
  • Crisis protocol developed participant-specifically
🏥 Complex Physical Referrals
For participants requiring Module 1 high intensity daily activities. Specify which supports are required — ventilator, PEG, tracheostomy, etc.
  • RN reviews feasibility before acceptance
  • Staff trained to NDIS HIDPA descriptors before first shift
  • OT environment assessment coordinated before move-in
